1. Services provided:
New Star Healthcare Service Inc. offers personal support services, including assistance with daily living, comfort care, and skilled care. Services are provided based on an agreed-upon care plan tailored to individual client needs.

2. Client Responsibilities:

• Provide accurate and complete personal and medical information.

• Treat our staff with respect and ensure a safe working environment.

• Notify us in advance of any changes to service requirements.
   

3. Payment & Billing:

• Clients agree to pay for services as per the agreed fee structure.

• Payments must be made on time, and any outstanding balances may result in service suspension.

• Cancellation fees may apply if services are cancelled without proper notice.

​4. Privacy & Confidentiality:
All personal information is handled following our Privacy Policy and applicable laws. We do not share personal information without client consent, except as required by law.

5. Service Modifications & Termination:

• We reserve the right to modify, suspend, or discontinue services at any time.

• Clients may terminate services with written notice, subject to applicable cancellation terms.

While we strive to provide high-quality care, New Star Healthcare Service Inc. is not liable for unforeseen incidents, damages, or losses resulting from service use, except in cases of proven negligence.

7. Governing Law:

These Terms and Conditions are governed by the laws of Ontario, Canada, and any disputes will be resolved in accordance with provincial legal procedures.
